Bereaved families and friends want lessons to be learnt so other families don’t suffer a similar loss.
There is currently no mechanism in place to share lessons, search or analyse PFDs, follow up responses to PFDs, or assess whether actions promised in PFDs are put into effect. The Preventable Deaths Tracker is trying to change that.
We are a small, dedicated team that has no core funding. With support, we could:
- regularly update the Tracker database and automate this process
- expand the Tracker database, making it more searchable and user-friendly
- build dashboards for real-time analyses
- rapidly identify trends and share these as preventable death “alerts”
- assess responses to PFDs to determine compliance with the statutory requirement of 56 days, creating a page for each respondent, showing their response rates and actions taken to prevent future deaths
- compare respondee performance to ensure accountability
- do more campaigning activities and training on PFDs
- deliver and respond faster to queries
